Choosing your doctor:


Selecting a primary care physician is very important and should be done as soon as you join Beaver Medical Group. Your primary care physician oversees all aspects of your health care including physical exams, preventive care and referrals to specialists.

Patients on an HMO plan must select a Beaver Medical Group primary care physician with their health insurance in order to use the services available at Beaver Medical Group. Simply call your health plan's membership number to make sure you insurance company assigns you to the Beaver physician of your choice. If you are an HMO member assigned to a primary care doctor outside of Beaver Medical Group, you cannot access our services. Usually you must request a physician transfer by the middle of the month for the change to be effective with your insurance by the first of the following month.

Family Practice, Internal Medicine and Pediatric physicians are available as primary care physicians.

You will receive an automated reminder phone call one or two days before your appointment at the primary telephone number you provided during registration. If for any reason you are unable to keep an appointment, please cancel or reschedule as soon as possible. This will allow other patients the opportunity to be seen sooner.

Medical Records

If you are a new patient, you will need to complete a medical records release form to have your chart transferred from your previous doctor to Beaver Medical Group. Please complete the form soon after you join Beaver Medical Group so your physician has your complete medical history. You may download our Medical Records Release form, fill it out and fax it to your previous doctor. Learn more about medical records.
Walk-in Care Available Every Day of the Year:

Beaver Medical Group's Urgent Care Centers provide convenient medical care for illnesses requiring immediate care. We are available to care for you and your family on a walk-in basis.

We treat patients of all ages and do not require a physician referral. Our Urgent Care Centers do not require an appointment and are available to everyone, even if you are not currently a BMG patient. Learn more about receiving urgent and after-hours care.

Health Education:

The Health Education Department offers a variety of health classes and programs, free to Beaver Medical Group patients. The mission of the Health Education Department of Beaver Medical Group is to educate, motivate, and empower patients and their support network, to take a proactive role in their own health care by making and maintaining healthy lifestyle changes.

On-site Services:

Laboratory, radiology, optical shops, Audiology, mammography and more are readily available to all patients. Learn more about on-site services.

Prescription Refills:

Allow 4-7 days for prescription refills. Call the pharmacy where you filled the prescription; the pharmacy will send a request to your doctor for a new authorization if you do not have any remaining refills. If you have not seen a Beaver Medical Group doctor yet and need a refill, call to make an appointment and bring all your medications with you.
